Donor Tracking Form - Download & Go the Extra Mile!
Go the "extra mile" to help homeless animals! Ask your friends and family to sponsor you and raise additional donations! A special FOWAS T-shirt will be mailed to donors raising over $100 (for our race registrants - $25 registration fee plus $75 in donations receive a bandana and T-shirt)!
Here's how:
- Download our Donor Tracking Form to document your cash and check donations.
- Check with donors to see if their employer has a Matching Gifts program!
- Include donor's contact info so they receive a written acknowledgment for their generosity (donations are tax-deductible).
- Submit your completed form along with donations:

Friends of the Warwick Animal Shelter is an all-volunteer, 501c3 humane organization - all donations benefit homeless animals at Warwick Animal Shelter.
