I am 18 years of age or older and would like to register for the Climb to End Cancer of Phoenix. I understand that this event is about more than just the miles we cover—it’s about empowering our community to come alongside the American Cancer Society in the mission to end cancer as we know it, for everyone.

To support this mission, all adult participants are encouraged to meet a minimum fundraising goal of $250 in addition to the $50 registration fee. As a thank-you for your efforts, once you reach that $250 milestone, you’ll be eligible to receive your $50 registration fee back—or apply it to your own fundraising total to boost your impact and earn ACS Rewards points at shopacsrewards.org.

I am between the ages of 18 and 23 and would like to register for the Climb to End Cancer of Phoenix as a student participant. I understand that this event is about more than just the miles we climb—it’s about empowering our community to join the American Cancer Society in the mission to end cancer as we know it, for everyone.

To support this mission, student participants are encouraged to meet a minimum fundraising goal of $100 in addition to the $25 student registration fee. As a thank-you for your efforts, once you reach that $100 milestone, you’ll be eligible to receive your $25 registration fee back—or apply it to your own fundraising total to boost your impact and earn ACS Rewards points at shopacsrewards.org.

I am 17 years old or younger and would like to register for the Climb to End Cancer of Phoenix with the permission of a parent or guardian. I understand that this event is about more than just a hike—it’s about empowering people of all ages to help the American Cancer Society end cancer as we know it, for everyone.

Youth participants are encouraged to meet a minimum fundraising goal of $50 in addition to the $15 registration fee. Once that goal is reached, you’ll be eligible to receive your $15 registration fee back—or apply it to your fundraising total to help earn ACS Rewards points at shopacsrewards.org.

I am registering as Living Proof — someone who has personally faced cancer as a survivor, thriver, or previvor — and would like to be celebrated as part of the Climb to End Cancer of Phoenix.

As Living Proof, I will receive a special edition white event shirt that distinguishes me as someone who inspires this movement and embodies our mission to end cancer as we know it, for everyone.

If I would also like to receive the standard participant shirt (non-Living Proof design), I may do so by adding $10 to my registration. This small contribution helps extend the impact of every dollar raised, supporting the American Cancer Society’s lifesaving work in our community and beyond.

We recognize this is a change from past years. Please know it was carefully considered to help streamline event operations, improve efficiency, and ensure every dollar goes further in the fight against cancer.
