Offline Donation Forms
If you would like to raise money for Pamoza offline, we have two offline donation forms. A half-page form that lets you collect information for one person at a time and a full-page form that lets you collect information from multiple donors at a time. Decide on which form you would like to use, then use it to collect information from your supporters.
Donations can be given to you at the time you are asking or after you complete the 5K. Checks should be made out to Pamoza and should have your name and 5K in the memo so that we can give you credit for the donation. Please bring the forms and checks or money you collected with you to the 5K or mail them to P.O. Box 23804, Philadelphia, PA 19143.
